OBS (Obstructive defaecation syndrome)

ODS is the inability of a person to evacuate the rectum normally, mostly female predominant and usually associated with genitourinary prolapse. A Colorectal Surgeon can usually be able to diagnose the underlying problem. For complex etiology and psychological involvement, ODS need multidiciplinary approach . Treatment should be specific and low cost.

Clinical features :

Excessive strain during defecation.
Incomplete evacuation of stool with repeated visit to the toilet.
Need perineal support or manual removal of stool to empty the rectum.
May or may not be associated with constipation .
Often associated with accidental bowel leakage.
Sometimes fecal incontinence and constipation overlap.
Most of the cases, patients are laxative dependant and psychologically handicapped.

Anal Fissure

Anal fissure is a common anal problem. Tearing at the anal margin is a painful condition characterised by pain during defaecation along with per anal bleeding. Burning sensation in anus is the characteristic feature of anal fissure. Sometimes itching aggravates the situation.

Cause of Anal Fissure

Constipation
Strain during defecation
Diarrhea
Anal Sex
Obstructed defecation
Idiopathic

Diagnosis of Anal Fissure:

Physician can diagnosis anal fissure by inspection of the anus or by gentle digital rectal examination.

Management of Anal Fissure:

Aim of Management of anal fissure is to lower the pressure in rectum.

Conservative treatment

Correction of constipation by more fluid and fibe, laxatives.
Correction of Diarrhea if any.
Warm hip bath and cleaning of anal region.
Avoid straining and prolonged sittings on the toilet.
Use of painkillers / lubrication.
